Please find attached our New Patient bundle of forms. Click here to obtain the New Patient package. Kindly read the forms, fill them out, and print them out. When you arrive at your appointment, bring the filled-out forms. We have set aside a specific time for your appointment, so please attend on time. However, in order for us to enter your information into our system before you are seen, we ask that you arrive fifteen minutes early for your initial appointment.

Payment is payable at the time of treatment unless other arrangements are made in advance. Click here to view our Financial Arrangements policy.

Financial Arrangements

We are pleased to electronically file your insurance claims and deliver the check to you in four weeks. Therefore, unless you have a credit history with our office, we will assume that all treatment will be paid for at the time services are provided.

Visa, Mastercard, American Express, and Discover cards are all accepted.

Alternatively, you can give us permission to receive the check, but you will need to have established credit with our office. In order to accomplish this, we will request that you designate us to retrieve the outstanding insurance balance from your credit card.
